CNA Job Summary

A paraprofessional person who is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN). Services are furnished in accordance with Agency policies and procedures.


Responsibilities:


  • Provide in-home personal hygiene and other care to assigned home health patients based on established care plans
  • Document the services provided
  • Understand and adhere to established Agency policies and procedures
  • Perform personal care, bath and other hands-on care as assigned
  • Complete appropriate visit records in a timely manner as per Agency policy
  • Report changes in the patient’s condition and needs to the RN
  • Perform household services essential to health care in the home as assigned
  • Ambulate the patient as assigned
  • Perform simple procedures as an extension of the therapy or nursing services, e.g., range of motion (ROM) exercises as assigned
  • Attend in-service and continuing education programs as scheduled and necessary
  • Attend patient care conferences as scheduled
  • Perform other job duties as assigned



Requirements:


  • High school diploma or equivalent
  • Current Tennessee driver’s license and automobile liability insurance
  • Current certification for Certified Nursing Assistant; Must meet Medicare Conditions for Home Health Training program and competency
  • Willingness to work in home environments of patients who may have values or standards different from your own
  • Ability to turn, lift and carry patients when necessary
  • Ability to climb stairs and do some heavy lifting
  • Ability to both give and receive instructions, on the phone and in person
  • Ability to use the tools of a home care aide, which include blood pressure cuff, stethoscope, thermometer, gloves, gowns, aprons, shampoo boards and medical records
  • Ability to maintain patient confidentiality and privacy
  • Ability to carry out directions, read and write
  • Have a sympathetic attitude toward the care of the sick and elderly
