PT Job Summary

Home care allows you to get a full understanding of every patient’s needs!

Duties include:


  • Initiating, evaluating, developing, implementing, documenting, supervising and revising plans of care for patients requiring physical therapy
  • Coordinating patient care with other staff members and providers
  • Documenting patient assessment, needs and care
  • Assisting with coordinating and teaching rehabilitative care programs to meet the needs of staff, patients, caregivers and the community
  • Supervising Physical Therapy Assistants

Requirements:

​

Home care is both challenging and rewarding! This job requires a person who can work independently, while never forgetting that home care is a team effort. Other requirements include:

​

  • Graduate of an approved school of physical therapy approved by the American Physical Therapy Association
  • One year of clinical experience as a physical therapist
  • Licensed as a physical therapist by the State of Tennessee
  • Knowledge of geriatric and pediatric problems
  • Ability to communicate effectively, both verbally and in writing
  • Respecting the patient’s privacy and maintaining confidentiality
  • Ability to turn, lift, carry and otherwise transport patients
  • Current Tennessee driver’s license and automobile liability insurance
  • CPR Certification
