PT Job Summary
Home care allows you to get a full understanding of every patient’s needs!

Duties include:
- Initiating, evaluating, developing, implementing, documenting, supervising and revising plans of care for patients requiring physical therapy
- Coordinating patient care with other staff members and providers
- Documenting patient assessment, needs and care
- Assisting with coordinating and teaching rehabilitative care programs to meet the needs of staff, patients, caregivers and the community
- Supervising Physical Therapy Assistants

Requirements:
​
Home care is both challenging and rewarding! This job requires a person who can work independently, while never forgetting that home care is a team effort. Other requirements include:
​
- Graduate of an approved school of physical therapy approved by the American Physical Therapy Association
- One year of clinical experience as a physical therapist
- Licensed as a physical therapist by the State of Tennessee
- Knowledge of geriatric and pediatric problems
- Ability to communicate effectively, both verbally and in writing
- Respecting the patient’s privacy and maintaining confidentiality
- Ability to turn, lift, carry and otherwise transport patients
- Current Tennessee driver’s license and automobile liability insurance
- CPR Certification
